Headwaters Feedback Form

Please use the form below if you wish to provide feedback on any Headwaters Farm or staff-related matter. You have the option to share your feedback anonymously. If the feedback is regarding EMSWCD’s Executive Director, your submission will go directly to Chris Wallace Caldwell of Catalysis LLC., who is a neutral third party and who facilitated the creation of the HIP Community Agreements.

If the feedback is regarding the Executive Director, Chris Wallace Caldwell will:

Reach out for additional information if contact information was provided.
If the feedback is anonymous, Chris Wallace Caldwell will determine if providing the information to an EMSWCD staff member will resolve the issue and address the concern.
If the feedback is regarding more serious issues and creates an obstacle for the farmer that cannot be resolved by an EMSWCD staff member, Chris Wallace Caldwell will convey the information to the EMSWCD Board Chair.
